Our core services
The Servant's Heart Corp offers comprehensive, tailored services to meet the diverse needs of people experiencing homelessness in Fort Collins, Colorado. Our approach is focused on understanding and responding to what individuals require, providing effective and practical solutions that lead to lasting change.
Street outreach & immediate assistance
Our Saturday night street outreach teams meet people where they are – in encampments, under bridges, and other unsheltered locations. We provide immediate, life-sustaining support including water, food, hygiene kits, basic supplies, and crisis connection, building crucial trust.
Advocacy & navigation services
We remove systemic barriers by assisting with critical paperwork and navigation. This includes obtaining identification, Social Security and disability benefits, SSI/SSDI applications, and navigating medical appointments, landlord communications, and housing authority processes.
Transitional housing & pathways
Beyond emergency shelter, we offer pathways to stability, like our Farmhouse Transitional Housing program that combines living stability with work therapy and skills building. We also provide 55+ senior-friendly rental options such as the 47th Place apartments.

Why our approach works
We understand that survival comes first. Food, water, socks, and safety are the baseline that keeps people alive long enough to make decisions. When someone isn’t in crisis mode, change becomes possible. Our consistent street outreach builds credibility and trust, turning reluctance into willingness to engage with services and appointments. Without trust, nothing else happens, and we make trust our currency.
Building stability, restoring purpose
Our navigation support removes system barriers, from obtaining IDs for benefits to securing housing for medical stability and work. This is why our ID help, SSI/SSDI navigation, and paperwork advocacy are life-changing. Transitional housing, like our Farmhouse model, creates actual stability, providing routine, personal responsibility, recovery support, and accountability. Our farm-based approach restores dignity by giving individuals, especially veterans, purpose through work therapy, animal care, and land stewardship, accelerating recovery and reintegration.
